Here we see four identically cut pieces of 3/4" Birch plywood that will make up the face plates (or fascia) for the Fork Mount Head. Two for the front, and two for the back. To conserve wood, I have cut out four separate tines to form the front and back fascia plates, rather than two 2-tine sections. Each set of tines (front and rear) will be mated to the Right Ascension Axis Support Box separated by  a 8 1/2" x 11 1/2" spacer.
